To initiate a claim, please click here to access a document and form entitled New Claim Form Requirements. In order for us to facilitate your claim in the least amount of time, we recommend you read all the instructions first.
At the bottom of the form you will find an area that will allow you to enter specific information pertaining to your claim. Please provide all the necessary requested documents. Once you have verified the accuracy of the information you are providing, please indicate the type of claim (accident, theft, fire, storm, or other) and then press SUBMIT.
You may need to provide additional information depending on the nature of your claim. Please click the appropriate claim form(s) you will need from the list below. In order for us to facilitate your claim in the least amount of time, please complete the form accurately and in its entirety.
When you download the appropriate product claim form, you will have two choices. First, you may print the form on your desktop printer, fill in the necessary information by hand, and mail or fax it to us. Or, second, you may download the form and open it in an Adobe Acrobat Professional Program. This will allow you to input or type the information requested directly into the form. Because not all computers will allow you to save the file in which case you will need to print it as soon as you have completed it. Before entering all the information, we recommend filling in your name and testing your computer’s ability to save it.
